Care Corner: The MaxHealth Impact
Reducing social isolation is key for patients to remain out of the hospital and reduce depression. At 90 years old, MaxHealth patient has no family that lives in the area and lives alone. This patient has a neighbor that will occasionally bring her to appointments, but they’re out of town for a few months each year. The patient recently needed medication, but the cut off time for delivery that day had passed. Office staff at MaxHealth Ellenton went to the pharmacy to pick up medication and hand deliver it to the patient’s home. While they were there, they checked the patient’s mailbox and opened the driveway gates to help her better access her mail since she has difficulty walking. Ellenton office manager, Jill LaPearl, makes transportation arrangements for the patient to be seen in the office and arranges diagnostic imaging at a location near the patient’s home. Case Manager, Melanie Britt, regularly checks in with her to see how she’s doing and helps coordinate appointments and other needs with the office. The MaxHealth Ellenton staff went above and beyond for this patient having a positive impact on her health, happiness, and overall well-being.
