Care Corner: The MaxHealth Impact
Being afraid of losing your house and personal items is a common fear amongst our elderly community. One of our patients with multiple advancing stages of chronic conditions was struggling with maintaining her independence at home with her husband. After multiple visits to the hospital, she was faced with having to move from her home to a skilled nursing facility. Our Nurse Care Manager, Carmen, along with Dr Yeli planned frequent visits to the office and weekly outreach calls to help her accept and understand her illness progression. Our Care Manager and Physician helped the patient and family by reducing anxiety and stress that many families experience at this critical stage of illness. We helped to guide the patient on the value of Hospice services to help her maintain her independence at home as her illness progresses. We were able to help this family understand the value of the supportive care that Palliative care and Hospice can provide at home.
