Notice of Data Breach: One of our vendors, TriZetto Provider Solutions (“TPS”) experienced a cybersecurity incident that affected protected health information (“PHI”) of certain Max Health patients. The compromised information related to Max Health’s patients potentially includes patient names, addresses, social security numbers, and insurance policy numbers. The incident notably did not affect any payment card, bank account or other financial information. TPS advised that on October 2, 2025, TPS learned that TPS’ system was accessed through a web portal utilized by TPS’ health care customers beginning in November of 2024, and TPS quickly conducted an investigation into the incident and engaged external cybersecurity expert, Mandiant, to mitigate the incident. Max Health understands TPS eliminated the threat and notified law enforcement, but we still encourage those impacted by the incident to diligently monitor possible incidents of identity theft and fraud, including by reviewing account statements and monitoring credit reports for suspicious activity.
