This page is for individuals who are currently enrolled in a Medicare Advantage plan and want to understand how their coverage works with primary care services MaxHealth.
At MaxHealth, we provide primary care services and general Medicare education to help patients understand how their existing Medicare coverage works with care at our clinics. We do not provide insurance advice, recommend plans, or enroll beneficiaries in Medicare plans.
If you are currently enrolled in a Medicare Advantage plan and reviewing your coverage during the Open Enrollment period, or looking for a primary care provider who accepts your existing plan, our Patient Advocates can help answer general questions about accessing care and, upon request, share contact information for independent, licensed insurance professionals to assist you concerning plan-specific information.

Below you’ll find all the essential tools to streamline Medicare Open Enrollment with confidence.
The Medicare Advantage Open Enrollment Period (January 1–March 31) gives current Medicare Advantage beneficiaries the opportunity to review their coverage and make a one-time change, such as switching to another Medicare Advantage plan or returning to Original Medicare, based on their individual health care needs for the year ahead. MaxHealth does not assist with plan changes or enrollment, but we can connect you to independent, licensed Medicare insurance professionals who can further assist.
As your Primary Care Provider, we wanted to remind you that the Medicare Open Enrollment Period (OEP) is underway. During this time, current Medicare Advantage beneficiaries may review their health care coverage options for the upcoming year.
We encourage you to review your plan’s Annual Notice of Change and visit Medicare.gov or call 1-800-MEDICARE for unbiased information to help you understand your options. MaxHealth does not provide plan comparison or recommendations.
Our practice continues to participate in many Medicare Advantage and Original Medicare plans; however, plan participation and provider networks may change from year to year.

It’s the time (Jan 1-Mar 31) where individuals who are currently enrolled in a Medicare Advantage plan may review their coverage. During this period, beneficiaries may make a one-time change, including switching to a different Medicare Advantage plan or returning to Original Medicare.
Not necessarily! Whether you can continue seeing your current doctor depends on the specific plan’s provider network. MaxHealth works with many major plans. However, because Medicare Advantage plan networks and provider participation can change, beneficiaries should review plan details carefully.
Some Medicare Advantage plans may offer additional benefits, which may include extras like fitness, dental, vision, food card programs, caregiver support, and more. Availability of these benefits varies by plan and location and are not included in all plans. You can visit Medicare.gov, call 1-800-Medicare, or speak with a licensed Medicare insurance professional to further discuss such benefits.
Just fill out the form or call our Patient Advocates at (866) 371-0838, and we’ll help you get scheduled at a MaxHealth clinic near you.
