Dear Patients –
MAXhealth is following the direction of the Florida Department of Health and CDC and encouraging patients to STAY HOME if you are sick with flu like symptoms. We are working closely with the local Department of Health and Hospitals to facilitate testing. Until you are tested, SELF-ISOLATION is the best way to protect yourself, your family, and the community.
All MAXhealth offices are open during regularly scheduled hours. During this time, all patient cancellation fees will be waived. For the safety of our staff and all patients seeking routine care, all patients will be screened outside the facility or placed in isolation.
IF YOU ARE NOT FEELING WELL – CALL BEFORE YOU COME IN
If you have or had a Fever (100.4 or greater), Cough, and Shortness of Breath within the last 7 days, please do NOT come to the office. This includes all scheduled office visits, lab appointments, diagnostic imaging or physical therapy. CALL the PCP office or our Care Management Hotline at 941-259-8499 for instructions.
The Department of Health COVID-19 Hotline is available 24/7 at 1-866-779-6121 to help screen and coordinate testing.
If you are SICK (Fever (100.4 or greater), Cough, Shortness of Breath)
CALL your MAXhealth Primary Care Provider IMMEDIATELY. If you are unable to speak to the Physician/Physician Assistant or Nurse Practitioner, CALL our Care Management Hotline at 941-259-8499. Or contact the Department of Health COVID-19 Hotline.
HOW TO SELF-ISOLATE
¨ Stay home and get medical care telephonically
¨ Separate yourself from other people and animals in your home
¨ Avoid sharing personal household items
¨ Cover your coughs and sneezes
¨ Wash your hands often
¨ Clean high-touch surfaces every day with disinfectant
The Heath and Well-Being of all our Patients is our PRIORITY. While patients with flu like symptoms are being asked to avoid our offices, we remain in contact and closely monitor them. If you have flu like symptoms and have NOT contacted your PCP, we encourage you to contact us.
